New Service Checklist --Print out this handy checklist to make the process as easy and smooth as possible.

Electrical Service Extension Request ("E" Form) -- This is the basic form, and often the only form needed for new service or changes to existing service.

Preliminary Estimate Request ("P" Form) -- Use this form to find out how much your large commercial project will cost.

Area Lighting Service Request ("L" Form) -- Use this form to request outdoor lighting on ML&P poles.

Facility Relocation Request -- This form is a contract to have ML&P relocate electrical facilities at customer cost.
